This role involves clinical triage for patients, monitoring the Electronic Health Record (EHR) nurse pool, and responding to incoming calls, messages, and EHR tasks in a timely manner. The position requires exercising nursing judgment and utilizing standard triage guidelines to assess and meet patient health needs. Responsibilities include assessing complex patient needs, ensuring appropriate level of care transitions, and triaging/scheduling visits based on patient disposition needs and available resources. The role also involves providing clinical support, answering phone calls and electronic messages, educating patients on community services, and collaborating with the interdisciplinary care team. Additional duties include greeting and escorting patients, obtaining vital signs, inputting clinical information into the EHR, assisting clinicians with procedures, resolving patient concerns, participating in meetings, and staying current with community resources. The position may also involve clerical support such as scheduling appointments, registering patients, scanning records, and preparing medical records. The ability to float to other practices and triage walk-in patients is also required, along with maintaining communication for safe transitions and continuity of care.
