This role involves monitoring the Electronic Health Record nurse pool, triaging incoming patient communications (calls, messages, In-Basket tasks), and prioritizing urgent matters. The position requires utilizing nursing judgment and standard triage guidelines to assess patient needs, ensure they are met, and transition them to the appropriate level of care. Responsibilities include collecting patient data, assessing responses, scheduling visits, and providing clinical support to the team. The role also involves answering phone calls and electronic messages promptly, educating patients about community services, and collaborating with the interdisciplinary care team. Additional duties include greeting and escorting patients, obtaining vital signs, inputting clinical information into the EHR, performing medication reconciliation, assisting clinicians with procedures, resolving patient concerns, participating in meetings and trainings, and staying current with healthcare system resources. Clerical support such as scheduling, registering patients, scanning records, and faxing may also be required. The position may require floating to other practices and verifying clinician orders for tests and procedures. The role also involves triaging walk-in patients and maintaining communication for safe transitions of care.
