Under RN supervision, the LPN, Continuous Care is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. This role involves providing nursing tasks to patients and families such as continuous care, dressing changes, and catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG). The LPN demonstrates clinical skills regarding application of Company policies and procedures such as medication administration, symptom management, and patient/family relationships. They observe/monitor patients for alterations in body systems, pain and symptoms, reporting to the RN, and observe and report signs of adverse reactions and responses to treatments to the RN. Findings are reported to physicians, APRNs, and physician’s assistants following collaboration with the RN. The role includes performing venipuncture for blood specimens and Central Venous Line (CVL) site observation/inspection, venous access device dressing, cap change and flushing, and IV medication and fluid administration, including IV pushes (per procedure) and venipuncture. The LPN may not make any changes to the infusion pump. Documentation is performed per policy and procedure in the medical record. During emergencies, the LPN may be required to report to work at a designated location to ensure continuity of services, which may include reporting ahead of schedule and staying overnight. Some locations may require cross-training to Hospice House positions for flexible staffing.
