Hospice & Community Care provides quality care to thousands of patients and families every year so they can live better at end of life. The organization supports hundreds of individuals in the community daily through its incredible staff. This LPN or RN Palliative Coordinator role makes a meaningful difference to patients and families. The position is PRN (as needed) with weekday shifts of 4 or 8 hours. After 6 months, there is an openness to a work-from-home discussion. A full-time orientation is required.
