About The Position

Hospice & Community Care provides quality care to thousands of patients and families every year so they can live better at end of life. The organization supports hundreds of individuals in the community daily through its incredible staff. This LPN or RN Palliative Coordinator role makes a meaningful difference to patients and families. The position is PRN (as needed) with weekday shifts of 4 or 8 hours. After 6 months, there is an openness to a work-from-home discussion. A full-time orientation is required.

Requirements

• RN or LPN
• 2 years of recent nursing experience required
• Certification CHPN/CHPLN within 2 years from hire date required
• Must be able to do a full time orientation
• Must be vaccinated for influenza or be granted a religious/medical exemption

Responsibilities

• Works in collaboration with the Choices interdisciplinary team, overseeing day to day team and patient schedules, patient tracking, care needs, evaluating referred clients, facilitating admission, coordinating care among providers and other team members, providing nursing services both in person and virtually, and assessing and addressing the ongoing needs of patients and caregivers, in order to ensure the effective delivery of palliative care to the patient and family.
