About The Position

Fallon Health's Summit ElderCare® is a Program of All-Inclusive Care for the Elderly (PACE) for individuals 55 and older, designed to help them live safely at home. The SE Licensed Practical Nurse will be an integral part of the Summit ElderCare Care Team, ensuring participants receive high-quality and timely care and support in the PACE center and the community. This role involves carrying out nursing tasks assigned by the RN Manager and SE Providers, with a focus on appropriate follow-through and timely documentation.

Requirements

Graduate of an Accredited Practical Nursing Program.
Valid license to perform Practical Nursing in the Commonwealth of Massachusetts.
Certification in CPR/Healthcare Provider issued by The American Heart Association or other accredited agency.
Must possess a valid driver's license and have a satisfactory driver's record report.
Must maintain the minimum state required insurance on the vehicle used while performing company business.
Must have reliable transportation.
Must meet the requirements of Fallon Health's Drivers’ Safety Policy.
At least three years of nursing experience in a primary care practice, long-term care facility, or similar clinical setting.
At least one year of nursing experience working with frail elders.

Nice To Haves

Willingness to occasionally assist other SE PACE centers either in person or remotely, as appropriate, when there is an opening or a gap in coverage.

Responsibilities

Implement plans of care formulated by the Provider and Primary RN in collaboration with the Interdisciplinary Team.
Carry out nursing tasks as assigned by the direct supervisor and in accordance with the nurse's license.
Receive calls from participants and caregivers, triage appropriately, and conduct follow-up with timely documentation.
Administer prescribed medications to participants attending the PACE center.
Record vital signs.
Carry out treatments and wound care as assigned for participants attending the day center.
Make frequent home visits in the community to evaluate the home environment.
Fill weekly medication packs.
Deliver relevant health teaching for participants and their caregivers.
Make recommendations to the Interdisciplinary Team (IDT) regarding unmet participant needs.
Coordinate care for SE participants with supervisors and other nursing staff.
Travel throughout the service area to participants’ homes and contracted facilities as needed.
