About The Position

We are seeking a compassionate and detail-oriented licensed practical nurse to work full-time in a office setting while providing high-quality in-home and community-based patient visits. This role is ideal for a nursing professional who values patient-centered care, strong clinical judgment, and effective communication. The successful candidate will support individualized care plans, perform routine nursing tasks, and help ensure each patient receives safe, respectful, and reliable care.

Requirements

  • Current and active practical nursing license in good standing.
  • Strong assessment, observation, and documentation skills.
  • Ability to work independently while remaining collaborative with the care team.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple visits and prioritize tasks effectively.
  • Basic computer skills and comfort using electronic documentation systems.
  • Current CPR certification or ability to obtain it as required.

  • Experience in home health, clinic, long-term care, or a similar patient care setting preferred.

Responsibilities

  • Conduct patient visits in accordance with care plans, agency policies, and applicable regulations.
  • Provide skilled nursing services such as vital sign monitoring, medication support, wound care, and other assigned treatments.
  • Observe, assess, and document patient conditions, progress, and any changes in status.
  • Communicate promptly with supervising clinicians, case managers, and other members of the care team.
  • Educate patients and caregivers on basic care instructions, safety, and health maintenance as directed.
  • Maintain accurate and timely clinical documentation.
  • Support continuity of care by following established protocols and reporting concerns appropriately.
  • Promote a safe, respectful, and patient-focused care environment.
