About The Position
Kline Galland Benaroya Home Health Services is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the comfort of patients' homes throughout King County. This float position serves patients throughout Central District and North King County. As a Home Health LPN, you will work closely with patients, families, and the interdisciplinary care team to deliver skilled nursing services and support individualized care plans.
Requirements
- Knowledge of infection control policies and procedures, i.e., Standard Precautions, Isolation Precautions, personal protective equipment, hand hygiene.
- Ability to work in an interdisciplinary team, supervised by a nurse
- Knowledge of the delivery of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Current Washington State LPN or Multistate LPN license
- 6 months of LPN experience
- Ability to drive with clean driving record
Nice To Haves
- Home health/hospice experience preferred
Responsibilities
- Visit patients in their homes and provide skilled nursing care according to the established plan of care.
- Implement physician-ordered and clinician-directed care plans.
- Monitor and evaluate the effectiveness of patient care and report changes in condition.
- Accurately document patient assessments, treatments, and medical history.
- Provide emotional support, education, and encouragement to patients and their families.
- Complete and submit timely progress notes and visit documentation daily.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure quality patient outcomes.
- Promote patient safety, comfort, and independence.
