About The Position

Optimal Care is seeking a dedicated Home Health Registered Nurse to join their team. This role offers the opportunity to practice nursing with autonomy, time, and patient connection, making a meaningful impact daily. The Home Health Registered Nurse will deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own environment. This role is ideal for an RN who values clinical independence, critical thinking, and building strong relationships with patients and families over time. The nurse will manage a caseload of patients from admission through discharge, providing skilled nursing care, conducting assessments, and developing individualized care plans tailored to each patient’s needs. Working closely with physicians and an interdisciplinary care team, the nurse will coordinate services, make informed clinical decisions, and play a central role in shaping patient outcomes and supporting each person’s journey toward greater health and independence. The role requires daily travel within the service area (Grand Rapids & Surrounding Areas) to provide care to patients in their homes, assisted living facilities, or nursing homes.

Requirements

  • Graduate of an accredited Nursing School: ASN or BSN
  • Current State License as a Registered Nurse
  • Minimum 1 year of experience as a licensed RN in a clinical care setting
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
  • Willingness to travel daily for home visits within service area

  • Experience in home health environment
  • OASIS certification or willingness to obtain
