We're seeking a detail-oriented and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care team as an MDS Coordinator. In this role, you'll help ensure the accuracy and timeliness of resident assessments while supporting quality care, regulatory compliance, and appropriate reimbursement. Working collaboratively with our interdisciplinary team, you'll play an essential role in developing individualized care plans that promote the best possible outcomes for our residents. The MDS Coordinator is responsible for coordinating, completing, and maintaining Minimum Data Set (MDS) assessments in accordance with federal and New York State regulations. This position works closely with nursing, rehabilitation, social services, dietary, and other departments to ensure comprehensive resident assessments and person-centered care planning.
