About The Position
Our client is seeking an LPN/LVN for long-term care in Madison, Wisconsin. The position is a 3x12 day shift. Candidates must hold current state licensure and have at least one year of recent experience. Applicants must provide the listed credentials and screening documentation and pass pre-employment, background, and urine drug screenings.
Requirements
- Current state licensure
- At least 1 year of recent experience
- Current resume
- Current BLS and/or ACLS and/or specialty certifications
- Two current clinical references
- Physical examination completed within the past 12 months
- TB skin test completed within the past 12 months
- Titers for MMR, Hepatitis B, and Varicella
- Respiratory fit test completed within the past 12 months
- Current Tdap and flu vaccinations
- Must pass pre-employment screening, background screening, and urine drug screening
