About The Position

The Temp Community Health LPN provides and documents nursing care to patients in their homes. The role promotes health, prevents illness, supports rehabilitation, and coordinates care with other healthcare professionals. The nurse must work independently, exercise sound clinical judgment, and demonstrate strong critical thinking, communication, and time-management skills.

Requirements

• Minimum of 1 year of acute nursing experience.
• Home health experience required.
• Active Massachusetts LPN license in hand at submission.
• Current BLS certification through the American Heart Association or American Red Cross.
• Valid driver’s license, auto insurance, and personal vehicle required.
• Knowledge of home health reimbursement, regulatory guidelines, and community health practices.
• Ability to work independently, maintain confidential records, and accept on-call responsibilities.
• Strong clinical judgment, critical thinking, communication, and time-management skills.
• Flexibility with work hours, scheduling, and holiday coverage.

Nice To Haves

• Hospital-based experience preferred.
• Certified Home Health experience and OASIS proficiency highly preferred.
• Infusion therapy experience preferred.
