About The Position

Under the direction of the On Call RN/Triage RN, this role supplements the nursing care needs of the patient as provided by the RN and as specified in the Plan of Care (POC). The position involves providing home visits as directed by the On Call RN/Triage RN to observe and report findings to the appropriate RN, and assisting the RN in carrying out the POC. The role also assists the patient in learning appropriate self-care techniques and provides services in accordance with organizational policies and procedures and regulations that define the scope of services. Additionally, it assists the RN and/or physician in performing specialized duties related to end-of-life care, requires timely submission of all documentation, and performance of other duties as required. Ensures overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

Requirements

  • Graduate of an accredited school of vocational nursing.
  • Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse.
  • Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting.
  • Experience in hospice or home health preferred.
  • Current State Drivers License.
  • Must have reliable transportation and provide ongoing valid and current auto liability insurance.

Nice To Haves

  • Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse preferred
  • Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting preferred, hospice or home health preferred

Responsibilities
