About The Position

Sage Group HealthCare Staffing is seeking an experienced LTC LPN/LVN for a travel/contract position in Claremont, New Hampshire. This full-time role involves providing direct resident care in a long-term care setting, administering medications and treatments, observing residents for changes in well-being, and supervising LNAs. The position requires a commitment of 40 hours per week with available shifts including evenings and days, and may require holiday coverage. The contract length is 13 weeks.

Requirements

Reside beyond a 50-mile radius of the facility location.
3 years of recent/current LVN/LPN experience (recent within 6 months).
LTC experience.
Willing to float.
Covid Vaccination Required.
LPN/LVN License for State of NH or compact.
BLS Certifications from AHA.

Nice To Haves

IV Certification (Required upon offer during onboarding).
Rural/small facility experience.
Prior travel or contract assignment experience.

Responsibilities

Observe each resident on a continuing basis alert to changes in mental and physical well-being; notifies the Supervisor of pertinent observations and discusses the need to notify physician.
Receive Resident Status Report from off-going shift and give on-coming shift a detailed report of each resident’s status and events of the shift.
Accurately receive, transcribe, and carry out physician’s orders.
Administer medications and treatments as ordered by the physician and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
Order and maintain all pharmaceuticals for the units.
Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing.
Assign and measure nursing staff resident care consistent with abilities, job description and scope of practice and supervise the care given.
Assist direct-care staff with nursing care as necessary and as time allows.
