About The Position

Encompass Health is hiring an LPN/LVN to provide compassionate, personalized care to patients in an inpatient rehabilitation setting. The role supports patients in pursuing their rehabilitation goals by delivering care aligned with individualized care plans and physician orders, monitoring their condition and treatment responses, and working with Registered Nurses. The position emphasizes building relationships with patients and supporting their physical, mental, and emotional needs throughout recovery.

Requirements

• Current LPN/LVN licensure as required by state regulations.
• CPR certification.

Nice To Haves

• One year of experience in an inpatient medical-surgical or general hospital setting is preferred.
• Rehabilitation experience is preferred.

Responsibilities

• Provide direct patient care in accordance with personalized care plans and physician orders.
• Observe patient behaviors and monitor vital signs, symptoms, and responses to treatments.
• Collaborate with Registered Nurses to report findings and carry out patient care plans.
• Build meaningful relationships with patients and understand their physical, mental, and emotional needs during recovery.
• Recognize and celebrate patient progress and achievements.
