LPN/LVN for RPM (Bilingual, Spanish Speaking)
About The Position
RPM Healthcare is seeking a bilingual (Spanish) speaking RPM 365 Care Coach who will engage patients, track and analyze their physiological data (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation), identify dangerous out-of-range values and trends, provide education, and encourage adherence to their care plans. The ideal candidate is a licensed LPN/LVN passionate about healthcare innovation and patient outcomes, tech-savvy, comfortable with high-volume phone outreach (125+ conversations a week), and thrives in a fast-changing environment. They should be patient, empathetic, detail-oriented, and a self-starter.
Requirements
- Active nursing license.
- Bilingual, Fluent in Spanish
- Knowledge of chronic conditions and related monitoring devices.
- Knowledge of remote patient monitoring CPT code qualifications.
- Proficiency with technology, computer platforms, and smartphones.
- Strong verbal/written communication skills (English and Spanish)
- Comfortable with patient outreach and engagement (English and Spanish)
- Highly organized, detail-oriented, and team-focused.
- Independently motivated
- Comfortable spending the majority of your workday engaging with patients over the phone providing support, education and guidance.
Nice To Haves
- Experience in telehealth, remote patient monitoring, education, and/or customer service.
- Experience in weight management coaching
- Additional language skills are a plus.
Responsibilities
- Serve as the primary contact for patients on the RPM365 platform.
- Assist with device setup and app connectivity.
- Engage with patients to ensure adherence to care plans.
- Schedule monthly check-ins and monitor patient data.
- Educate patients through shared resources.
- Notify healthcare providers of critical alerts.
- Attend virtual team meetings for updates.
