About The Position

Our 87-bed, Medicare-certified skilled long-term care facility offers opportunities to build relationships with residents and families while applying nursing knowledge. The Care Center provides care across long-term care, a specialized dementia unit, and a transitional care unit. Your work will help residents maintain their abilities, receive personalized care, and feel respected in their home.

Requirements

Graduation from an accredited nursing school.
Current Minnesota LPN licensure.
BLS Healthcare Provider certification.
Completion of continuing education required to maintain LPN licensure.

Responsibilities

Provide personalized nursing care according to residents’ care plans.
Complete focused assessments within your scope of practice, recognize changes in condition, and consult the RN when needed.
Administer and document medications and treatments safely and accurately.
Receive, transcribe, and carry out provider orders as directed by the RN.
Maintain timely daily charting and communicate resident needs to the care team.
Participate in care conferences and share observations that support resident care.
Coordinate assignments with the RN, assist with staff coverage when necessary, and follow up on assigned care.
Help team members understand nursing procedures and policies, including assisting with new employee orientation.
Respond thoughtfully to questions from residents and families or connect them with the appropriate person.
Follow infection prevention and safety practices and respond appropriately to emergencies.
