This role focuses on helping residents maintain their abilities, receive personalized care, and feel respected in their home.

Requirements

  • Graduation from an accredited nursing school.
  • Current Minnesota LPN licensure.
  • BLS Healthcare Provider certification.
  • Completion of continuing education required to maintain LPN licensure.

Responsibilities

  • Provide personalized nursing care according to residents’ care plans.
  • Complete focused assessments within your scope of practice, recognize changes in condition, and consult the RN when needed.
  • Administer and document medications and treatments safely and accurately.
  • Receive, transcribe, and carry out provider orders as directed by the RN.
  • Maintain timely daily charting and communicate resident needs to the care team.
  • Participate in care conferences and share observations that support resident care.
  • Coordinate assignments with the RN, assist with staff coverage when necessary, and follow up on assigned care.
  • Help team members understand nursing procedures and policies, including assisting with new employee orientation.
  • Respond thoughtfully to questions from residents and families or connect them with the appropriate person.
  • Follow infection prevention and safety practices and respond appropriately to emergencies.
