About The Position
We are seeking a dedicated Licensed Practical Nurse (LPN) for a Long Term Care (LTC) assignment in Santa Fe, NM. The ideal candidate will provide high-quality nursing care to residents in a skilled nursing and long-term care environment while collaborating with the interdisciplinary healthcare team to support patient safety and well-being.
Requirements
- Active New Mexico LPN license or active multistate compact license required
- Current AHA BLS certification required
- Minimum of 2 years of recent LPN experience required
- Strong communication, clinical, and organizational skills
- Ability to work independently and as part of a healthcare team
- Commitment to delivering compassionate and professional patient care
Nice To Haves
- Previous Long Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred
Responsibilities
- Provide direct nursing care to long-term care residents in accordance with individualized care plans
- Administer medications, treatments, and nursing procedures as prescribed
- Monitor resident conditions and promptly report changes to the supervising RN or physician
- Maintain accurate and timely patient documentation
- Assist with activities of daily living (ADLs) and resident support needs
- Promote a safe, clean, and therapeutic environment for residents and staff
- Collaborate effectively with nursing staff, providers, therapists, and support personnel
- Follow infection control, safety, and facility compliance standards
- Deliver compassionate, patient-centered care during night shifts
