The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. This role demonstrates knowledge of nursing principles for the resident in the relevant clinical setting and practices within the full scope of nursing expertise/knowledge. The LPN utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan. They possess the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. The LPN demonstrates the ability to recognize problems, recommend solutions and respond effectively to assist in an emergent situation, ensuring the health, comfort and safety of residents. They contribute to the assessment of residents and administer medications or treatments as ordered, and provide technical support to healthcare professionals as needed. Documentation of resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary is essential. The role requires skill in establishing and maintaining effective working relationships with residents, health care providers, and the public, and the ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team. Communication clarity and maintenance of confidentiality of sensitive information are key. The LPN will counsel residents, family members and/or resident advocates, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans, and possess knowledge of resident education principles to develop a self-care program. Other related duties as assigned.
