About The Position

This is a full-time, benefit-eligible position for an LPN Inpatient working 36 hours per week on 12-hour night shifts. The role involves providing direct nursing care, implementing interventions, and administering medications as prescribed, all while collaborating with an interdisciplinary team and adhering to the hospice philosophy. Responsibilities include patient assessment, education, documentation in an EMR system, and participation in quality improvement initiatives. The LPN will maintain patient confidentiality and may be exposed to infectious diseases.

Requirements

  • Graduate of an accredited school of practical nursing with preferably two (2) years clinical experience; one year (1) in acute care setting.
  • Current license as a Licensed Practical Nurse in Ohio.
  • Verification of successful completion of a medication course.
  • Has the ability to work in collaboration with the interdisciplinary team.
  • Has working experience charting in an EMR system, Netsmart system preferred.
  • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Must be CPR certified according to the American Heart Association CPR standards.
  • Understands and is committed to the hospice philosophy.

  • Netsmart system experience preferred.

Responsibilities

  • Performs direct nursing care, nursing interventions and prescribed treatments based on established plan of care (POC) and hospice policy and procedures as delegated by the RN.
  • Observes and reports patient’s physical, emotional and spiritual needs as well as changes in the patient’s condition and response to treatment plan to the RN.
  • Administers medications according to physician orders and hospice polices and procedures.
  • Obtains and processes physician orders.
  • Provides appropriate education and information to patients, families, and caregivers related to the patient’s plan of care and needs.
  • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication and performance of duties.
  • Participates in the agency’s Quality Improvement program.
  • Attends staff meetings and completes all mandatory training.
  • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
  • Performs other duties as assigned.
