About The Position

Hospice of the Piedmont is seeking a dedicated and skilled Licensed Practical Nurse to provide personal care services to Hospice House patients. This role involves providing compassionate care to individuals in need and supporting patients and families during their end-of-life journey.

Requirements

• Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia.
• Two years’ relevant nursing experience required.
• Ability to manage all aspects of terminal care, health team service coordination, and documentation.
• Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families.
• Basic to intermediate proficiency in word processing software.
• Driver’s license, personal automobile, and current automobile liability insurance.

Responsibilities

• Provides nursing care to Hospice residents and oversees nursing assistants.
• Works with the team to identify patient care needs.
• Reports changes in residents' conditions to relevant medical staff.
• Maintains accurate patient records.
• Assists with food preparation, housekeeping, and completes various administrative tasks.
• Follows personnel policies and maintains safety and infection control standards.
• Manages medication orders, performs narcotic counts, and ensures proper medication handling.
