According to prescribed policies and procedures of the organization including all applicable federal, state and accreditation regulations and reporting to the Clinical Leader, the Licensed Practical Nurse is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth by the plan of care. The role involves providing direct patient care, implementing the plan of care initiated by the registered nurse, and ensuring accurate and timely documentation. The LPN will also provide patient and family education, participate in the coordination of home health services, and mentor direct care staff under the supervision of the RN. This position requires participation in on-call rotations and interdisciplinary team meetings. The ideal candidate will demonstrate a positive, professional manner, manage time efficiently, and collaborate effectively within a team. Adherence to all organizational policies, procedures, and ethical standards is expected, including respecting resident rights and reporting any complaints or allegations of abuse. Completion of annual education requirements is also mandatory.
