About The Position

As a Licensed Practical Nurse (LPN), you’ll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Requirements

• Graduate of an accredited practical/vocational nursing program
• Current LPN/LVN license in the State of Arizona
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

• 3 years of nursing experience preferred (Home Health or MedSurg a plus)

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations
• Implement care plans developed by the RN and document progress accurately
• Monitor patient conditions and communicate observations to the RN and physician
• Collaborate with the care team to ensure seamless, high-quality service
• Participate in professional development and team meetings
