About The Position

This full-time position involves supporting patient care delivery as part of the home health team, working under the direction of the Clinical Manager. The role requires assisting with patient care activities for a designated group of patients, supporting the delivery of care aligned with Advocate Health at Home standards, helping ensure patients receive high-quality, cost-effective care, promoting and supporting patient and family rights, and contributing to maintaining and improving overall quality outcomes. Travel is required throughout Near North City and North Suburbs.

Requirements

• Active license in the State of Illinois
• Minimum of 2 years of medical-surgical (med/surg) experience
• Comfortable using computer-based scheduling and documentation systems
• Strong written and verbal communication skills
• Effective interpersonal and teamwork skills
