About The Position

This Home Health RN position serves patients throughout Chicago's North Side and Lakefront communities. The territory includes neighborhoods from Rogers Park and Edgewater south through Uptown, Lakeview, Lincoln Park, and Downtown Chicago, with travel concentrated along the I-90 corridor and surrounding communities. Nurses manage a geographically aligned caseload, providing in-home care to patients across a diverse urban service area. This position includes a rotating schedule with weekend, holiday, and on-call responsibilities. Team members work every third weekend and participate in an on-call rotation approximately three times per month on weekdays (5:00 PM – 8:00 AM). Additional on-call coverage is required every sixth scheduled working weekend. Holiday requirements include one winter holiday and one summer holiday each year.

Requirements

Nurse, Registered (RN)
A valid driver’s license issued by the Division of Motor Vehicles.
Graduate from an accredited school of nursing
Earn 10 continuing education credits annually
One year of medical surgical experience
Has as demonstrated proficiency for the requirements for RN role
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR
