This position assumes responsibility for caseloads consisting of patients in specialty areas. The RN Case Manager conducts initial assessments to obtain thorough physical, social, and environmental data. They establish the plan of care in accordance with physician orders and patient needs, providing direct patient care and adjusting nursing care processes as needed. The role involves identifying problems, modifying nursing care plans, and evaluating outcomes. The RN Case Manager advises and consults with family and other personnel, provides ongoing assessment of patient response to treatments and teaching, and updates the plan of care as indicated. They are responsible for completing documentation accurately and promptly, communicating with physicians to confirm and update care plans, and facilitating the discharge planning process. Participation in chart audits, continuous quality improvement activities, and process improvement opportunities is also expected. The role requires maintaining and updating accurate clinical and patient records according to agency, state, and federal guidelines. The incumbent must demonstrate knowledge and skills necessary to provide age-appropriate care, understanding principles of growth and development and assessing data reflective of patient status to meet age-specific needs.
