About The Position

Mass General Brigham Home Care, Inc. is seeking an LPN to join their Logan team. This role focuses on providing 1:1 skilled nursing care to patients in their homes, supporting recovery, independence, and chronic illness management. The position emphasizes autonomy, meaningful patient relationships, and practicing at the top of one's license. Responsibilities include delivering complex clinical treatments and educating patients to prevent hospital readmissions. Primary coverage areas include Chelsea, East Boston, Revere, and Winthrop. The role offers a full-time schedule, Monday-Friday, with an average of 6-7 visits per day, including dedicated time for documentation and care coordination. No hospital shift chaos, overnight rotations, or weekends are involved.

Requirements

  • LPN professional license in Massachusetts
  • Minimum 1-year previous nursing in acute care or sub-acute care setting required
  • Effective written, verbal and electronic communications skills
  • Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred
  • Ability to communicate effectively in writing, verbally and electronically
  • Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population
  • Strong interpersonal skills with ability to work with varied population
  • Ability to work as a member of an interdisciplinary team
  • Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
  • Travel required in local geographic area

Responsibilities

  • Deliver 1:1 skilled nursing care

  • Provide quality nursing care to patients in various settings in the community
  • Provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures and standards of practice.
