About The Position
University Home Care Inc is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality home care services to clients in the Southeast MI Region. Must be willing to travel. As an LPN for Home Care, you will play a critical role in supporting patients' health and well-being by delivering personalized nursing care in the comfort of their homes.
Requirements
- Valid and current LPN license in the state of Michigan.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Reliable transportation to travel to patient homes in the Southeast MI Region.
Nice To Haves
- Previous experience in home care or related clinical setting preferred.
Responsibilities
- Provide direct nursing care to clients in their homes according to care plans and physician orders.
- Monitor and record patients’ health status and report changes to healthcare providers.
- Administer medications and treatments as prescribed.
- Assist with activities of daily living and educate patients and their families on health management.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain accurate and timely documentation of care provided.
