About The Position

Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. This role will serve patients located in West Cuyahoga and Lorain County. Cleveland Clinic Homecare provides skilled nursing, social services and physical, occupational and speech therapy for patients returning home after a hospital stay, outpatient procedure or doctor’s visit. This team continues Cleveland Clinic treatment at home and helps patients learn or re-learn any skills necessary to stay safe and independent at home. As a Licensed Practice Nurse, you will visit and provide care to approximately 6 patients each day in the comfort of their own homes. LPNs in this role typically start and end their day working remotely. A caregiver in this position works Monday – Friday from 8:00AM – 5:00PM, every 4th weekend and one summer and one winter holiday per year. There are no on-call requirements for this position.

Requirements

  • Graduate from an accredited practical or vocational nursing program.
  • Current Ohio license as a Licensed Practical Nurse.
  • One year of clinical experience as a professional nurse in an acute setting.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage.
  • Knowledge of JCAHO and Medicare standards and regulations.

Nice To Haves

  • M-IV certification.
  • Infusion experience.

Responsibilities

  • Ensure continuity of care and coordinate discharge planning.
  • Act as an interdisciplinary team member by providing direct patient care under multiple levels of care.
  • Collect and document objective and subjective patient data, in accordance with the agency policies and procedures.
  • Report data, as appropriate, to the directing registered nurse and other members of the health care team, so that a plan of care is initiated, and changes are accurately addressed.
  • Actively engage with and educate potential customers about the benefits and appropriateness of care.
  • Ensure the patient and their family have adequate knowledge and an understanding of the hospice or homecare benefit, as appropriate.
  • Contribute to the assessment of patients by collecting data for analysis by an RN.
  • Observe patients and report any unusual conditions to professional staff.
  • Observe patients for safety risks and implement safety measures that are within scope of practice.
  • Administer medications, within scope of practice, and observe patients for effectiveness in relief of symptoms as well as for adverse reactions to medications or treatments.
  • Provide direct patient care, including dressing changes and laboratory tests, as part of the plan of care.
  • Provide ongoing education and information to patients and family members, taking into consideration cultural issues and other special challenges.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
  • Ensure a seamless transition to the residential setting by coordinating services at time of discharge.
  • Be flexible in setting and revising priorities for one’s daily assignment based on patient needs/acuity, family requests and available resources.
