About The Position

Mass General Brigham Home Care, Inc. is seeking an LPN to join their Minuteman team. This role focuses on delivering 1:1 skilled nursing care to patients in their homes, supporting recovery, independence, and chronic disease management. The position emphasizes autonomy, meaningful patient relationships, and practicing at the top of one's license. Responsibilities include delivering skilled nursing care, collaborating with RN Case Managers, providing complex clinical treatments, and supporting patients recovering from surgery, illness, or chronic disease exacerbations. The role aims to prevent hospital readmissions and improve patient lives through hands-on clinical care and education. Patient care is delivered within MGB Home Care's philosophy, policy, and standards of community health practice. In collaboration with the RN Case Manager, this position will provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.

Requirements

  • LPN professional license in Massachusetts
  • Minimum 1-year previous nursing in acute care or sub-acute care setting required
  • Effective written, verbal and electronic communications skills
  • Ability to work with various computer software required
  • Ability to communicate effectively in writing, verbally and electronically
  • Strong interpersonal skills with ability to work with varied population
  • Ability to work as a member of an interdisciplinary team
  • Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
  • Travel required in local geographic area
  • Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population.

Nice To Haves

  • Home care experience preferred
  • knowledge of hand-held patient care computer devices preferred
