About The Position

Mass General Brigham Home Care, Inc. is seeking an LPN to join their Blue Hills team. This role focuses on providing 1:1 skilled nursing care to patients in their homes, supporting recovery, independence, and chronic illness management. The position emphasizes autonomy, meaningful patient relationships, and practicing at the top of one's license. The LPN will collaborate with RN Case Managers, provide complex clinical treatments, and focus on hands-on care and education to prevent hospital readmissions. Primary coverage areas include West Roxbury and Roslindale, with support for Dedham, Hyde Park, and Westwood.

Requirements

LPN professional license in Massachusetts
Minimum 1-year previous nursing in acute care or sub-acute care setting required
Effective written, verbal and electronic communications skills
Ability to work with various computer software required
Ability to communicate effectively in writing, verbally and electronically
Demonstrated knowledge and skills necessary to provide care to the geriatric, pediatric, and/or adult population
Strong interpersonal skills with ability to work with varied population
Ability to work as a member of an interdisciplinary team
Strong organizational skills, ability to work independently and adapt to unpredictable circumstances
Travel required in local geographic area

Nice To Haves

Home care experience preferred
knowledge of hand-held patient care computer devices preferred

Responsibilities

Deliver 1:1 skilled nursing care

Provide skilled nursing visits, utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures and standards of practice
