About The Position

Fairview Range is hiring an LPN to join our Home Care and Hospice Team in Hibbing, MN. As a Licensed Practical Nurse under the direction of a Registered Nurse, you will play a critical role in coordinating and delivering high-quality, patient-centered care to individuals receiving services in their homes or in hospice settings. This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the day shifts with no weekends. The facility is a Medicare Certified Agency and the care team includes RNs, Physical Therapists, Occupational Therapists, Social Workers, LPNs, HHAs, Chaplain, Volunteers, Physician Medical Director and a Nurse Practitioner. Requirements

  • Graduate of an accredited LPN program
  • 1 year acute, long term care, or home care experience
  • Experience with Windows based medical records documentation
  • Licensed Practical Nurse in Minnesota
  • Basic Life Support (American Heart Assoc or Red Cross) within 90 Days
  • Current MN driver's license and auto insurance

Responsibilities

  • Participates, in collaboration with RN, in gathering ongoing assessment data including physical, psychological, and spiritual data.
  • Demonstrates awareness of age-specific criteria in obtaining assessment data.
  • Identifies client needs in collaboration with a registered nurse and incorporates into an individualized plan of care.
  • Assists the RN in coordinating a plan of care based on prioritized patient needs in collaboration with the client, significant other and resource personnel.
  • Understands expected outcomes identified for the client and works with the health care team in meeting these outcomes.
  • Provide age specific direct patient care in collaboration with RN's and all involved disciplines according to standards of care.
  • Directs paraprofessionals in the implementation of care.
  • Accepts additional assignments as requested by Home Care Supervisor.
  • Recognizes and reports adverse client condition to the case manager and/or physician in a timely manner.
  • Evaluates the clients' response to treatment based on identified outcomes and informs the case manager of unexpected findings.
  • Documents clinical and/or supportive data into the medical record per policy.
  • Assesses client/family readiness to learn.
  • Provides an environment conducive to learning.
  • Assists with education of paraprofessional staff.
  • Understands scope of practice as defined by statue and HealthLine Policy and provides patient care within defined parameters acknowledging strengths and weaknesses.
  • Maintains productivity levels established by the organization.
  • Participates in ongoing informational and/or educational activities related to clinical knowledge and professional issues.
  • Demonstrate ability to be organized while managing frequent interruptions.
  • Capable of functioning well under pressure and in stressful situations.
  • Able to give, receive, and analyze information, articulate goals, and action plans, demonstrate effective communication skills, and manage stress.
  • Able to understand and follow complex written and oral communication.
