Home Helpers Home Care is seeking an exceptional LPN Home Care Coordinator to join their clinical and client care team. This part-time role involves combining nursing knowledge with client support, caregiver orientation and training, field visits, care coordination, and quality assurance. It is an opportunity for an LPN who enjoys people, problem-solving, and teaching, with potential for growth as the company expands. Requirements

  • Current Georgia LPN license
  • Strong clinical judgment and communication skills
  • Comfortable teaching, coaching and assessing caregiver skills
  • Organized, dependable and detail-oriented
  • Comfortable working independently in the field and collaboratively with a team
  • Takes initiative and follows through
  • Interested in learning, growing and taking on additional responsibility

Nice To Haves

  • Home care, home health, senior care, assisted living, skilled nursing or related experience preferred
  • Exceptional LPN with transferable experience and the right approach to people

Responsibilities

  • Conduct client home visits, wellness visits and follow-up
  • Assist with new client starts and changes in care
  • Monitor client conditions and communicate concerns to our clinical team
  • Assist with new caregiver orientations, training and skills assessments
  • Provide caregiver education, coaching and clinical support
  • Help ensure caregivers understand and follow client care plans
  • Support caregiver competency and skills development
  • Communicate with clients, families, caregivers and our internal team
  • Review documentation and support quality assurance and regulatory compliance
  • Provide hands-on clinical support within LPN scope of practice when appropriate
