About The Position

The LPN (Foster Care) - Nurse will be responsible for providing comprehensive nursing care to children in foster care, adhering to NYSDOH and ACS guidelines. This role involves managing electronic health records, coordinating medical and dental appointments, developing health plans, and providing health education to children and foster parents. The LPN will also be responsible for maintaining accurate medical records, making referrals, and ensuring compliance with all regulatory requirements.

Requirements

  • LPN currently licensed by the New York State Education Department.
  • Proof of vaccinations required.

Responsibilities

  • Give immunizations and follow NYSDOH guidelines.
  • Work within an electronic health record.
  • Manage and follow ACS guidelines and 29I requirements for children in foster care.
  • Perform physical, mental health, and developmental assessments of children upon admission.
  • Arrange for initial and annual comprehensive physical examinations by a medical provider.
  • Arrange for initial and annual dental and optical examinations by agency contracted providers.
  • Schedule appointments with primary care providers and appropriate medical specialists.
  • Maintain comprehensive electronic medical records for each assigned child.
  • Develop a written descriptive health plan for each child in conjunction with the medical provider.
  • Oversee consent issues for procedures and treatment.
  • Monitor compliance with time frames for appointments and follow-up services.
  • Develop health histories and abstracts for children being transferred to another agency or schools.
  • Provide discharge summaries for youth aging out of foster care.
  • Obtain family health history from birth parents in conjunction with the caseworker.
  • Report all illnesses, accidents, and hospitalizations as required by the regulatory agency.
  • Contact medical providers, hospital clinics, and laboratories for medical reports.
  • Attend staff meetings and case conferences when indicated.
  • Review medical records, laboratory, and medical reports.
  • Provide psychiatrists with medical information, including laboratory results, for monthly medication review.
  • Provide health education to youth on preventive health and safety measures.
  • Provide training to patients and foster parents on topics related to preventive health and safety measures.
  • Provide counseling to adolescents on HIV-related information, family planning, and other sexually transmitted infections, and document these discussions.
  • Create, implement, and maintain a care plan for each identified problem.
  • Provide daily management and coordination of appointments, supplies, and medication for foster children on caseload.
  • Act as a resource to other staff members involved in patient care.
  • Receive visit summaries from all medical appointments and document them in the record.
  • Review and make all follow-up appointments.
  • Provide training for foster children and staff on medical conditions, health education, proper medication administration, safety precautions, universal precautions, age-appropriate anticipatory guidance, and other training based on the care plan.
  • Demonstrate competency in nursing practices, including proper nursing skills, use of medical equipment and terminology, and employment of current nursing protocols and procedures.
  • Make appropriate referrals for dental, hearing, and vision screening and other problems, and follow up in a timely manner.
  • Maintain medical records in an updated (real-time), organized, and legible condition, including medications.
  • Perform other related duties as requested by supervisor/Director.
