This position supervises Resident Care and Medication Assistants, ensuring residents receive required support according to their Individual Service Plan, completing daily procedures, ensuring resident safety, and meeting all care needs in compliance with Department of Health regulations and community policy.

Requirements

  • Ability to proficiently read, write, speak, and understand the English language.
  • Ability to make independent decisions when circumstances warrant.
  • Ability to deal tactfully and effectively with internal and external customers.
  • Knowledgeable of policies and procedures, as well as DOH regulations pertaining to assisted living levels of care (ALR, EALR, SNALR).
  • Valid state of residence driver’s license.
  • Current LPN License.
  • Reliable transportation.
  • Dependable, report to work on-time for every scheduled shift and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO).
  • Willing and able to work overtime and weekends as required.
  • Clean and neat appearance with a "friendly," hospitality-oriented, and service-oriented demeanor.
  • Abide by work-related/job procedures and organization policies.

Nice To Haves

  • Two (2) years of experience as a licensed Practical Nurse in a care facility or other related health care facility.

Responsibilities

  • Assisting with planning, developing, organizing, implementing, evaluating, and supervising resident care services, including medication assistance in accordance with current rules, DOH regulations, and guidelines.
  • Ensuring Resident Care Assistants (RCAs) are aware of job duties and supervising the completion of all duties.
  • Meeting with administration, medical and resident care staff, and other departments to plan resident care.
  • Interpreting care policies and procedures to staff, residents, and family members.
  • Maintaining service records, resident files, and progress notes.
  • Making written and oral reports or recommendations to the Nurse Manager concerning care services.
  • Maintaining an adequate liaison with families and residents.
  • Noting and reporting errors in the medication supervision program.
  • Reporting resident complaints or grievances to the Nurse Manager.
  • Creating and maintaining an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Providing guidance and training for new and/or less experienced staff.
  • Making appropriate reports to the Nurse Manager regarding departmental performance evaluations.
  • Identifying and correcting problem areas and/or improving services.
  • Reporting occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to the Nurse Manager.
  • Reviewing resident care needs and physicians' orders throughout each shift.
  • Assisting residents with ADLs as necessary and encouraging resident participation.
  • Attending and participating in workshops, seminars, and in-services to stay current with healthcare changes.
  • Ensuring staff, residents, and visitors follow established safety policies and procedures.
  • Encouraging residents to participate in ADLs based on their level of care (ALR, EALR, SNALR).
  • Ensuring staff follows established infection control procedures and that adequate supplies are available.
  • Ensuring staff is knowledgeable and follows residents' rights and HIPAA guidelines.
  • Assisting residents with prescribed injectable medications permitted by the Department of Health and assisting with blood glucose checks.
