To assess potential, actual and ongoing needs of the patient. To provide professional discharge planning, including, but not limited to discharge assessments, patient advocacy, resource management, utilization management, and community education, designed to meet the varying discharge needs of patients and their families as these issues relate to the current hospitalization or the subsequent transition from the acute hospital setting.
