North Alabama Medical Center is a 263 bed facility offering the community’s only open heart surgery site, a complete line of women’s and children’s services and a complete line of medical and imaging services. We are a Certified Chest Pain center and recognized as a Primary Stroke Center of Excellence by the American Heart/American Stroke Association. North Alabama Medical Center’s 24-hour emergency department serves as the primary referral center for major trauma cases in Northwest Alabama. Requirements

  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
  • Bachelor's Degree preferred or 3 years of experience

Responsibilities

  • Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care
  • Keeps track of all Medicare forms and consent forms
  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan
  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions
  • Assisting with group activities
