Under RN supervision, the LPN, Continuous Care is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. This role involves providing direct patient care, including medication administration, symptom management, and patient/family education, while collaborating with the RN and the Interdisciplinary Group (IDG). The position requires flexibility to meet patient needs, the ability to manage emotional stress, and perform essential job functions with or without accommodation. Responsibilities also include documentation, performing venipuncture, and managing central venous lines. In emergency situations, the LPN may be required to report to a designated company location to ensure continuity of services.
