About The Position
The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. In this rewarding role, you will provide compassionate end-of-life care and support to patients and families in the comfort of their homes. We offer comprehensive training and support, including a thorough orientation program, specialized training in end-of-life and hospice care, and dedicated preceptors and ongoing mentorship. We prioritize your safety with comprehensive training and necessary protective equipment. This role involves meetings and charting conducted via a supplied laptop, and national pre-taxed mileage reimbursement is provided.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- 1 Year LPN experience
- Valid driver's license and reliable transportation.
- Strong communication, documentation, and organizational skills.
Nice To Haves
- 1 or 2 years of homecare and/or Hospice experience preferred
- No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.
Responsibilities
- Provide skilled nursing care to hospice patients in the home setting.
- Monitor patient status and communicate changes to the interdisciplinary team.
- Support patients and families through education, symptom management, and compassionate care.
- Assist with implementation of the hospice plan of care.
- Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.
