About The Position
The Community Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice Home Care team. In this rewarding role, you will provide compassionate end-of-life care and support to patients and families in the comfort of their homes. We are committed to prioritizing your safety by providing comprehensive training and necessary protective equipment. This role involves providing skilled nursing care, monitoring patient status, supporting patients and families through education and symptom management, assisting with the implementation of the hospice plan of care, and collaborating with an interdisciplinary team.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- 1 Year LPN experience
- Valid driver's license and reliable transportation.
- Strong communication, documentation, and organizational skills.
Nice To Haves
- 1 or 2 years of homecare and/or Hospice experience preferred
- No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.
Responsibilities
- Provide skilled nursing care to hospice patients in the home setting.
- Monitor patient status and communicate changes to the interdisciplinary team.
- Support patients and families through education, symptom management, and compassionate care.
- Assist with implementation of the hospice plan of care.
- Collaborate with nurses, physicians, social workers, chaplains, home health aides, and other hospice team members to ensure quality care.
