About The Position
The Community Hospice has a rewarding opportunity for an LPN to work as part of our team caring for patients and families in their homes primarily in the Saratoga/Washington county area. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). Community Hospice provides a thorough orientation, preceptors and a supportive work environment.
Requirements
- Valid NYS LPN License
- Minimum 1 year LPN experience
- Strong computer skills
- Organization and communication skills and ability to work as part of a team
Nice To Haves
- Prior hospice/ home care experience preferred, but not required
- Training provided in end of life care.
Responsibilities
- Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families
