LPN - Community Hospice - Nursing Homes Team
About The Position
The Community Hospice is seeking a Full-Time Licensed Practical Nurse (LPN) to join our Nursing Homes team. This role is ideal for an LPN who values teamwork, relationship-building, and delivering compassionate end-of-life care. The position involves providing hospice care for patients residing in nursing homes throughout the Capital Region. As part of an interdisciplinary hospice team, the LPN will collaborate closely with Registered Nurses, Hospice Aides, Physicians, Social Workers, Chaplains, Complementary Therapists, and Volunteers to provide exceptional care and support to patients and their loved ones.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- 1 Year LPN experience
- Valid driver's license and reliable transportation.
- Strong communication, documentation, and organizational skills.
Nice To Haves
- 1 or 2 years of homecare and/or Hospice experience preferred
- Passionate about caring for others and are committed to learning.
Responsibilities
