About The Position
Full-Time LPN Hospice Nursing Home Team – Capital Region Schedule : Monday through Friday - 8:00 a.m. to 4:00 p.m. No on-call or mandated overtime! The Community Hospice is seeking a Full-Time Licensed Practical Nurse (LPN) to join our Nursing Homes team. Join a compassionate team dedicated to making a difference in the lives of patients and families! This rewarding role is ideal for an LPN who values teamwork, relationship-building, and delivering compassionate end-of-life care.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- 1 Year LPN experience
- Valid driver's license and reliable transportation.
- Strong communication, documentation, and organizational skills.
Nice To Haves
- 1 or 2 years of homecare and/or Hospice experience preferred
- No hospice experience? No problem. We welcome nurses who are passionate about caring for others and are committed to learning.
Responsibilities
- Provide hospice care for patients residing in nursing homes throughout the Capital Region.
- Collaborate closely with Registered Nurses, Hospice Aides, Physicians, Social Workers, Chaplains, Complementary Therapists, and Volunteers to provide exceptional care and support to patients and their loved ones.
