About The Position

This LPN position will provide care for clients in their homes, healthcare provider offices, on the mobile health unit, WIC clinic, public health offices, and more. This position is supporting mobile integrated health, decreasing transportation barriers for clients. The Licensed Practical Nurse (LPN) supports the Mobile Integrated Health (MIH) Program and GROW Initiative by providing community-based maternal and infant health services to pregnant individuals, postpartum mothers, infants, and women of reproductive age. This position focuses on improving access to care, reducing transportation barriers, addressing social drivers of health, and connecting families to healthcare services and community resources through outreach, education, navigation, and clinical support. The role supports maternal and infant health outcomes through home visits, community-based interventions, care coordination, and collaboration with healthcare providers and community partners. The position aligns with regional efforts to expand maternal health navigation, community outreach, telehealth utilization, and coordinated care services.

Requirements

  • Previous Healthcare experience is required
  • Active LPN License is required
  • Requires graduation from a nationally accredited licensed practical nursing program.
  • Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.
  • Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
  • Successful completion of orientation required.

Responsibilities

  • Provide maternal and infant health navigation and care coordination services.
  • Conduct intake activities, screenings, and follow-up visits.
  • Assist patients with establishing prenatal, postpartum, pediatric, primary care, and specialty care services.
  • Connect families to community resources and healthcare providers.
  • Coordinate referrals and follow-up to ensure successful connections to services.
  • Support patients experiencing barriers related to transportation, healthcare access, and social needs.
  • Complete SDOH assessments and screenings.
  • Identify unmet social needs and facilitate referrals to community resources.
  • Provide referral follow-up and resource navigation support.
  • Maintain knowledge of available community programs and support services.
  • Administer immunizations for women and infants in accordance with organizational policies and scope of practice.
  • Perform blood pressure screenings and other approved clinical procedures.
  • Conduct infant weight checks and maternal follow-up assessments.
  • Provide home safety education and screenings.
  • Assist with laboratory specimen collection and approved point-of-care screenings.
  • Support medication education and prescription coordination activities.
  • Connect eligible patients to Remote Patient Monitoring (RPM) services.
  • Assist with RPM enrollment, equipment setup, education, and follow-up.
  • Facilitate access to telehealth services and virtual specialty consultations.
  • Support expansion of telehealth utilization and maternal health services in rural communities.
  • Provide education on: Healthy pregnancy and prenatal care, Postpartum recovery, Infant health and development, Safe sleep practices, Breastfeeding and basic lactation support, Nutrition and healthy lifestyles, Parenting and infant safety, Chronic disease management during pregnancy.
  • Participate in community outreach events, parenting classes, and educational programs.
  • Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients.
  • Collaborates with other members of the healthcare team.
  • Evaluates response to interventions and patient status toward goal achievement.
  • Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.
