About The Position

Adoration Health is hiring compassionate nurses and Certified Nursing Assistants for private-duty, one-on-one care in patients’ homes throughout East, Middle, and West Tennessee. Caregivers support pediatric and adult patients, helping them live with dignity and independence while building meaningful connections with patients and families. The role offers flexible scheduling, independent work with clinical team support, and opportunities for advancement. Available schedules include full-time, part-time, and PRN; shifts include days, nights, overnights, and weekends.

The responsibilities listed are specifically for a Private Duty Licensed Practical Nurse. The nurse provides care according to each client’s plan of care, administers medications and other prescribed care, documents services and outcomes, communicates changes in condition, coordinates necessary supplies, and ensures patient needs are met in accordance with physician instructions.

Requirements

• LPN/LPT/LVN degree from an accredited college of nursing.
• Current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• Professional CPR certification.

Nice To Haves

• At least one year of nursing experience.
• Home Health care experience.

Responsibilities

• Provide one-on-one care according to the client’s plan of care while maintaining the client’s dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed under the client’s plan of care, and assess and report results.
• Accurately document nursing care, communications with family, case managers, and physicians, and progress toward established goals.
• Inform the physician, case manager, and family of changes in the client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings to maintain adequate amounts on hand.
• Ensure patient needs are met in accordance with physician instructions.
