About The Position

Lakewood Health System is seeking a part-time Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) to join our Care Management Team. Care Management is about making a difference beyond the clinic visit. Our team helps patients stay connected, supported, and on track with their healthcare goals by coordinating care, monitoring patient needs, and helping patients navigate their healthcare journey. This role offers a rewarding mix of clinical care, patient engagement, and care coordination while working collaboratively with providers and other members of the healthcare team. The LPN/CMA – Care Management supports high-quality, coordinated patient care by helping patients navigate their healthcare needs and remain engaged in their plan of care. This position works closely with patients, providers, and the broader healthcare team to monitor patient needs, maintain accurate care plans, coordinate services, and support quality initiatives. The goal is to help create a seamless patient experience while promoting continuity of care and positive health outcomes.

Requirements

  • Active Minnesota LPN licensure or current CMA/AAMA certification.
  • Current BLS Healthcare Provider certification.
  • Strong clinical and patient-care skills within scope of practice.
  • Proficiency in EMR documentation and workflow management.
  • Ability to prioritize tasks, problem-solve, and work independently.
  • Strong organizational skills and attention to detail in documentation and reporting.
  • Effective communication and collaboration with patients, providers, and healthcare teams.

Responsibilities

  • Assess patients within applicable scope of practice, monitor patient status, and communicate changes or concerns to providers.
  • Maintain accurate care plans and timely documentation within the electronic medical record (EMR).
  • Support patient rooming, screenings, intake, scheduling, and care coordination.
  • Monitor and respond appropriately to phone calls, MyChart messages, and provider in-baskets.
  • Assist patients with navigating appointments, follow-up care, referrals, and other healthcare needs.
  • Collaborate with providers and members of the healthcare team to support continuity of care.
  • Support Chronic Care Management (CCM), Health Care Home (HCH), and other quality initiatives.
  • Maintain accurate documentation and reporting related to care-management activities.
  • Help identify opportunities to improve patient engagement and coordination of care.
