About The Position

Phoenix Home Care and Hospice is hiring an LPN Clinical Manager to lead and support our Home Health team in Independence, MO. This role combines clinical leadership, field support, and care coordination in a fast-paced home health environment. The position is ideal for an experienced nurse who is seeking a leadership opportunity and is ready to step out of the routine. The role involves supporting and guiding field nurses and caregivers, assisting with care plans and clinical oversight, conducting supervisory and client visits, ensuring quality patient care and compliance, communicating with physicians, families, and the care team, and supporting scheduling, training, and clinical problem-solving. This is a leadership role where the successful candidate will be a reliable daily support for their team.

Requirements

• Active Missouri or Compact LPN license
• Home health experience preferred
• Strong organization and communication skills
• Ability to multitask and lead in a fast-moving environment
• Positive, team focused attitude

Responsibilities
