About The Position

Heritage Senior Living is seeking a compassionate and dependable Clinical Supervisor / Licensed Practical Nurse (LPN) to join their clinical leadership team. This position offers the opportunity to combine hands-on nursing with clinical leadership. The role involves working closely with the Director of Nursing to support resident care, oversee caregivers and CNAs, respond to changes in resident condition, and ensure residents receive safe, compassionate, and individualized care. Requirements

Current and valid Wisconsin Licensed Practical Nurse (LPN) license in good standing.
Knowledge of professional nursing principles and standards applicable to geriatrics, senior living, and long-term care.
Knowledge of or ability to learn applicable Wisconsin CBRF and RCAC requirements, including DHS 83 and DHS 89.
Ability to practice within the applicable Nurse Practice Act and LPN scope of practice.
Strong clinical judgment, organizational, and problem-solving skills.
Ability to provide direction, assign responsibilities, encourage staff, and promote positive resident outcomes.
Strong written and verbal communication skills.
Ability to function independently while collaborating effectively with the interdisciplinary team.
Ability to maintain positive relationships with residents and families.
Ability to manage multiple priorities and adapt to changing resident and operational needs.
Competency or ability to become competent in the safe use of applicable resident-assistance equipment, including floor lifts, sit-to-stand lifts, and friction-reducing devices.
Ability to travel as needed.
Ability to meet applicable pre-employment, training, licensing, and regulatory requirements.
Communicate effectively with residents, families, employees, physicians, and healthcare partners.
Read, understand, and accurately complete clinical records, physician orders, service plans, reports, and other required documentation.
Stand and walk for extended periods while providing resident care and supervising clinical operations.
Sit, bend, reach, stoop, kneel, push, pull, and perform movements necessary to provide resident care.
Regularly lift, carry, or move up to 50 pounds and safely assist residents using appropriate techniques and equipment.
Safely operate resident-assistance equipment, including mechanical and sit-to-stand lifts, as required.
Perform nursing and resident-care functions within the applicable LPN scope of practice.
Maintain regular and reliable attendance.
Work scheduled shifts and additional hours based on resident and community needs.

Nice To Haves

Senior living, assisted living, memory care, CBRF, RCAC, long-term care, or geriatric nursing experience is strongly preferred.

Responsibilities

Evaluates, plans, coordinates, and provides nursing care within the LPN's applicable scope of practice and in accordance with Heritage Senior Living policies and applicable regulatory requirements.
Provides nursing care based on resident needs, individualized service plans (ISPs), physician orders, and applicable standards of practice.
Administers medications and treatments in accordance with physician orders and applicable requirements.
Monitors residents for changes in physical, cognitive, behavioral, or emotional condition and performs appropriate resident evaluations.
Makes routine checks on residents who are acutely ill or experiencing a change in condition.
Assists the DON with developing, reviewing, and maintaining individualized service plans and updates ISPs as appropriate.
Maintains professional and compassionate relationships with residents and their families, protecting resident dignity, privacy, confidentiality, and property.
Completes appropriate follow-up related to changes in resident condition, physician notifications and orders, family/responsible-party notifications, laboratory results, accidents and incidents, skin alterations, new admissions, and discharges.
Reviews resident charts, progress notes, incident reports, and other clinical documentation for accuracy and completeness.
Oversees the 24-hour report and ensures significant resident concerns receive appropriate follow-up.
Communicates important resident information to the DON and appropriate members of the care team.
Collaborates with other departments to promote continuity and quality of resident care.
Provides clinical direction and support to caregivers and CNAs, monitoring resident care to ensure services are provided according to the ISP and applicable requirements.
Reviews caregiver and CNA documentation for accuracy, completeness, and appropriate follow-through.
Coaches, educates, counsels, and provides performance feedback to care team members as appropriate.
Assists with addressing employee performance or disciplinary concerns in collaboration with nursing and community leadership.
Assigns and coordinates tasks based on resident needs and staff responsibilities.
Provides education and training to care team members as needed.
Promotes teamwork, accountability, professionalism, and resident-centered care.
Maintains knowledge of applicable Wisconsin requirements for CBRF and/or RCAC settings, including DHS 83 and DHS 89 as applicable.
Practices within the applicable Nurse Practice Act and LPN scope of practice.
Maintains accurate, timely, and detailed documentation in residents' electronic medical records.
Assists with reviewing and investigating reportable, sentinel, accident, and incident events as assigned, participating in root-cause review and development of interventions.
Helps identify clinical trends, resident risks, and opportunities to improve quality of care.
Maintains high professional and ethical standards and represents Heritage Senior Living positively.
Partners closely with the Director of Nursing and other community leaders.
Participates in daily care-focused meetings and weekly care coordination meetings.
Attends and participates in leadership meetings as assigned.
Communicates effectively with nursing, caregivers, families, physicians, pharmacies, and other healthcare partners.
Assists the DON with clinical operations and other responsibilities as assigned.
