LPN - Clinical Intake Coordinator
About The Position
Bishop Spencer Place, a premier non-profit Life Care retirement community, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join their team as a Clinical Intake Coordinator. This role is crucial in facilitating the long-term care and rehabilitation process. The Clinical Intake Coordinator receives referrals from various sources, evaluates their appropriateness for long-term care and rehabilitation under Medicare, Medicaid, and commercial insurance regulations, determines the need for further information, communicates acceptance or decline of referrals, and processes accepted referrals. The position also involves supporting clinically related marketing activities and participating in an on-call rotation for referral management. Occasional weekend availability and local travel may be required.
Requirements
- Licensed Practical Nurse
- 2 years of applicable experience
Responsibilities
- Facilitates the long-term care and rehabilitation process by receiving referrals from multiple referral sources within and without the St. Lukes Health System.
- Performs initial evaluation of referral for long-term care and rehabilitation appropriateness under Medicare, Medicaid, and commercial insurance regulations.
- Determines need for additional information from referral sources.
- Communicates agency acceptance or decline of referral to varied referral sources.
- Processes accepted referrals.
- Supports clinically related Marketing activities.
- On-call rotation for referral management.
