Requirements

  • Graduation from an accredited school of nursing
  • Current, unrestricted LVN license valid in the state of employment
  • At least one year of nursing experience in a home health setting
  • Strong organization, communication, and attention to detail
  • Ability to manage multiple priorities in a fast-paced environment

Nice To Haves

  • Bilingual Spanish and English skills are a plus!

Responsibilities

  • Reviewing and clearing Homecare Homebase (HCHB) coordination notes
  • Processing administrative and clinical workflows accurately and on time
  • Obtaining verbal, written, and faxed physician orders and entering them for review and approval
  • Reviewing, updating, and maintaining the At-Risk Registry
  • Assisting with patient follow-up appointments, health summaries, pre-authorizations, and patient questions
  • Completing chart audits according to Elara Caring policies
  • Verifying receipt of Medicare Notice of Non-Coverage documentation
  • Protecting patient and employee privacy in accordance with HIPAA requirements
  • Supporting the branch with occasional field visits and direct patient care, as needed
  • Completing additional clinical or administrative projects as assigned
